Целта на дисертационния труд е да се проучи влиянието на тестостерон пропионат върху някои лабораторни показатели при модели на андрогенен дефицит (орхиектомия, възрастни животни) в експериментални условия на остро и хронично третиране. Основни задачи на проучването са: 1. Да се изследва ефекта на тестостерон заместителната терапия върху хематологичните показатели (еритроцити, хемоглобин, хематокрит, среден обем на еритроцитите, левкоцити и тромбоцити); 2. Да се проследи ефекта на тестостерон заместителната терапия върху показатели за метаболитни нарушения (тотален холестерол, триглицериди, серумна глюкоза); 3. Да се изследва активността на ензими, свързани с хепаталния статус (АлАТ, АсАТ, АФ, ЛДХ) по време на заместителната терапия с тестостерон пропионат; 4. Да се проследи вилянието на тестостерон заместителната терапия върху нивата на серумните протеини (тотален протеин, албумин); 5. Да се изследва динамиката в концентрацията на серумен калций при тестостерон заместителна терапия; 6. Да се проучи влиянието на тестостерон пропионат при заместителна терапия върху нивата на някои хормони: 6.1. Динамика в стойностите на тоталния тестостерон; 6.2. Концентрация на мелатонин в серума. 7. Да се проучи динамиката в стойностите на простатно специфичен антиген по време на заместителна терапия с тестостерон пропионат. ***** The problem about the male “menopause” was introduced for the first time during the 16th century in chineese texts on internal diseases. Since ancient times it was considered, that the testicles keep the man brisk and energetic. Years later Eugen Steiinach noticed, that the ligation of the testicular vessels causes atrophy of the germinative epithelium, as well as hypertrophy of the Leydig cells, and supposed that such interference could have a favourable influence. Lots of authors prove that there is a decrease of the testosterone level in the process of aging in men. Christ-Crain reports that there is a relative hypoginadism in each man over 60 years of age. This leads to changes in the body structure, lowered energy, muscle strength and physical functions, reduction of sexual functions, depressive mood, indisposition and decreased cognitive functions. Such changes are also seen in young men with androgen deficiency. The latter symptoms improve significantly after testosterone replacement therapy (TRT). Still the physiological and clinical importance of the age related decreases of the serum testosterone levels in men are not clear enough. This is partially due to the fact that the testosterone level in aging men is near the normal in young men. Practically seen it is most appropriate to define the term “andropause” as age related decrease of the serum levels of testosterone in aging male, compared with the level of the free testosterone in young male and associated with clinical syndrome (symptoms and syndromes related with androgen deficience). The decreased testosterone level can be a result of various diseases, as well as consequence of their treatment. The importance of the clarification of this problem is concerned with the improvement of the quality of life of the men with male sexual hormones deficiency. There is no data in the accessible literature about the influence of testosterone propionate in the replacement therapy upon the dynamics of the most often used in the rutine practice laboratory indexes. It is nessesary to study its effect in acute and chronic appliance upon the most accessible markers for the bone marrow, hepatic function and metabolic control and etc. The received results could be used as accessible indexes of the effect of testosterone 52 replacement therapy, which guide the specialists to more profound highly specialized or hormonal tests. The held experimental research of androgen deficiency includes different animal models of young castrated male wistar rats, simulatively (sham) operated rats and old rats. Haematologic, clinic-chemical and specific indexes were tested, which we concider important for the manifestation of androgen deficiency syndrome. Our research recommends observation of the calcium levels in TRT or treatment of osteoporosis. It was established for the first time an age related increase in the activity of LDH, which were normalized by the replacement therapy with testosterone propionate. The results of the current experimental study show that as in orchiectomized, as well as in aged male rats the application of testosterone propionate for 15 days or 15 weeks does not change the level of PSA towards controls. The application of testosterone propionate does not change the transaminase activity. The results received from the study can be used for prophilaxis and treatment of adverse drug reactions, related with TRT for determination of the dose, way of application and the drug form of testosterone. The correct diagnose and treatment of androgen deficiency will impove the quality of life of aged and young men with hypogonadism, will reduce the mortality by cardiovascular diseases and will improve their social adaptation.
