CLINICAL X-RAY DENSITOMETRY AND QUANTITATIVE ULTRASOUND OF BONE This book is focused on the most important problems of clinical bone densitometry and quantitative ultrasound. It shows the point of view of the clinician who is active in the field of metabolic bone diseases and osteoporosis. The contemporary definitions of osteoporosis are explained to the reader. Epidemiological data, both from literature and from original studies, are displayed. The value of bone mineral density in the assessment of fracture risk is well underlined. Two chapters discuss the clinical risk factors for osteoporosis as well as the important role of body weight. The OSTA-index, allowing fracture risk assessment from age and body weight, is displayed. Different clinical tools for fracture risk assessment such as questionnaires as SCORE, SOFSURF, ORAI etc. are critically presented. An originally developed questionnaire as well as its application in clinical practice are shown. The conventional X-ray in the diagnosis of osteoporosis is discussed in a whole chapter. This chapter is focused on X-ray morphometry of the vertebral bodies as well as on the Singh index. The forearm bone densitometry is critically discussed in the next chapter. The correlations with Dual-energy X-ray absorptiometry of the central sites are presented. Original patented methods for calculation of pure cortical and trabecular forearm bone density, as well as of volumetric forearm bone density are introduced. These methods are validated against the reference methods. Special attention is payed to the quantitative ultrasound. The strong and weak points of the different methodologies are discussed. The most commonly used in Bulgaria devices (Hologic Sahara, Lunar Achilles, Sunlight Omnisense) are introduced based on a variety of studies. The Dual-energy X-ray absorptiometry is the central point of a whole chapter. The principles of good clinical practice are displayed, as well as the strong and weak points of the methodology. These include quality control, reference databases and calculated T- and Z-scores. Die international consensus meetings in bone densitometry are listed, as well as the critical stages in the development of bone densitometry in Bulgaria. This book might be very helpful for medical staff and operators in the field of X-ray bone densitometry and quantitative ultrasound.
